Dear Participant,
You are invited to participate in a 10 minute survey to increase your awareness of ciguatera fish poisoning and have your say on future research directions.

What is the project about?
The purpose of this survey is to raise awareness about ciguatera fish poisoning among recreational fishers.

Who is undertaking the project?
This project is being conducted by Ms. Natalie Dowsett, Ms. Alison Turnbull and Dr. Andreas Seger.

Why am I being invited to participate?
You are being invited as you are a recreational fisher interested in information on ciguatera and a desire to influence future research directions.

What am I being invited to do?
You are being invited to answer short questions on your awareness of ciguatera fish poisoning. The survey is completely anonymous and no personal identifying information will be collected.

Are there any risks associated with participating in this project?
We understand that people who are or have suffered from ciguatera might experience discomfort by filling out this survey. Should you believe that this topic will cause you discomfort, please think carefully about whether you continue.

What are the potential benefits of the research project?
This survey will provide us with a better idea of the awareness of ciguatera amongst the recreational fishing community and help us increase this awareness.

Can I withdraw from the project?
Participation in this project is completely voluntary. If you agree to participate, you can withdraw at any time by closing the online survey. After you have submitted the full survey, we will not be able to withdraw your responses, as the data is collected and stored without personal identifiers.

What will happen to my information?
Information and data gathered from the study will be aggregated, stored in non-identifiable (anonymous), electronic form. The aggregated data will be shared in relevant scientific and fishing magazines, as well as via recreational fishing groups on Facebook and national and/or international conference presentations. Your information will only be used as described in this participant information sheet and it will only be disclosed according to the consent provided, except as required by law. The aggregated data collected in this study can be used in any future research.

What if I have a complaint or any concerns?
The study has been approved by the Human Research Ethics Committee at the University of Adelaide (approval number H-2019-234). This research project will be conducted according to the NHMRC National Statement on Ethical Conduct in Human Research 2007 (Updated 2018).
