Clevidipine, brand name Cleviprex, is emulsified dihydropyridine calcium channel blocker (CCB), concentration of 0.5mg/ml with a rapid onset (2min), short duration (5-15min), and organ independent esterase metabolism. It targets the arteries reducing systemic vascular resistance while preserving preload, and increasing cardiac output. Heart rate is preserved and may increase in a dose dependent manner as blood pressure is reduced. Clevidipine is administered as a continuous infusion starting at 1mg/hr titrating up by doubling the dose every 90 seconds to a maximum rate of 32mg/hr, however due to lipid load of 0.2g/ml, 24-hour average rate should not exceed 21mg/hr. The most common maintenance dose is 4-6mg/hr. Clevidipine has also been used in bolus doses of 0.125-0.5mg prior to induction and prior to bypass cannulation during cardiac surgery, and up to 1mg bolus doses to treat hypertension in pheochromocytoma patients to correct hypertension within 90 seconds.
