Dear Participant,

You are being invited to complete a survey about remote support (also known as remote monitoring) even if you are not using it. Remote support use electronic equipment to support and assist people with developmental disabilities in their home. This service is provided from an off-site location, typically using home and community based waivers to fund the cost and maintenance of equipment and staff.

Your answers are helpful, even if you have not used remote support. Your feedback will provide us with valuable suggestions for the improvement and expansion of remote support.

Project Title: University Support in Technology Evaluation and Development 

Purpose of the study: This research study aims to identify how technology, including remote support, can be used to promote independence for people with developmental disabilities. This means the technology that is identified may make it easier for an adult with developmental disabilities to have less dependence on others.

Procedures and time spent: First, you will be asked questions about yourself, including technology use. Then, you will be asked to give your opinion about remote support or identify why you have not yet used remote support. The time needed to complete the questionnaire is about 10-15 minutes.

Confidentiality: Your information will be anonymous. Unless you choose to provide contact information, no personal identifiable information will be recorded. Efforts will be made to keep your study-related information confidential. However, there may be situations where this information must be shared. For example, personal information about your participation in this study may be shared if required by state law. Also, your responses may be reviewed by the following groups (as applicable to the research): Office for Human Research Protections or other federal, state, or international regulatory agencies; The Ohio State University Institutional Review Board or Office of Responsible Research Practices; The sponsor (Ohio Department of Higher Education and Department of Development Disabilities).

Risks: We will work to make sure nobody sees your responses. However, when using the internet there is always a chance that someone could try to access information without authorization. If this happens the information could be used to identify you.

Benefits: Involvement in this study may result in an improved use of technology to promote independence for adults with developmental disabilities, including a remote support service that is improved and expanded beyond the home.

Incentives: No incentives are associated with study participation.
Remember, participation in this study is optional. This means you may choose to participate or not to participate in this study without penalty or loss of benefits to which you are otherwise entitled. If you choose to participate in the study, you may stop participating at any time without penalty or loss of benefits to which you are otherwise entitled.
