1. Consent Form

* 1. Dear potential participant:

We are graduate students at Mercy College. As part of research for our master's thesis in Speech-Language Pathology, we are conducting a survey to investigate the level of knowledge of teachers and speech-language pathologists in the differential diagnosis of Autistic Spectrum Disorder (ASD) and Attention Deficit Hyperactivity Disorder (ADHD). We are asking you to fill out this questionnaire to advance our research. Filling out the questionnaire should not take any longer than 20 minutes. You will not have to answer any questions that you do not wish to answer. Your participation will be kept confidential and completely anonymous. The questionnaire does not ask for your name, or other personal identification. Furthermore, we have no knowledge of any information you may have included for access to this website.

There are no anticipated risks, compensation or benefits to you as a survey participant. Your decision whether or not to participate will not affect your current or future relations with Mercy College. If you decide to participate, you are free to withdraw your consent to participate and may discontinue your participation in the survey without any consequences, however, once you submit your survey we will be unable to withdraw you from the research data since we will have no way of identifying you.
