Survey of Surveyors:
Re-Certification Survey & Complaint Investigation Process

The Vermont Division of Licensing and Protection (DLP) has asked for the Vermont Program for Quality in Health Care Inc. (VPQHC) to support an additional option for collecting feedback on the survey and complaint investigation process from hospitals, long-term care facilities, and home health agencies. While this new feedback option is available, DLP continues to encourage direct communication.

The survey is available to any staff involved in the survey/investigation process. No identifying information will be collected, and all results will be kept confidential and aggregated by VPQHC. The goal of this survey is to enhance the survey experience for both facilities and surveyors. It will not be used to penalize facilities or to evaluate individual performance on behalf of DLP.

At the end of the survey, participants will have the opportunity to provide additional comments or feedback on any items not covered, or to offer further clarification on previous responses.
