Thank you for taking the time to fill out the Bureau of Justice Assistance Law Enforcement / Mental Health program survey.

The survey is designed to collect detailed information about specialized police responses to people with mental illness. Only someone who is highly familiar with a specific, local police-based program should complete the survey.

While the survey is meant to examine law enforcement / mental health collaboration, some questions require an understanding of police department procedures; therefore, the survey is best answered by a representative of the law enforcement agency or a program coordinator with a detailed understanding of all aspects of its operation. The person responding to the survey is encouraged to consult with colleagues. If you would like to preview the survey in order to confer with colleagues before completing it, click here.

We ask for only one survey per program. If you are not the most-appropriate person to respond on behalf of your program, please forward the survey to this colleague.

Completing the survey should take approximately 30 minutes. Unfortunately, this version does not allow you to save your responses and return later.
