Healthy Volunteer Application

THANK YOU for applying to become a healthy volunteer and help sick patients in need. Please fill out the form below with your contact information and we will send you a detailed health survey to determine if you qualify as a healthy volunteer. Volunteers that qualify can make up to $4800 a month for their participation!

In order to become a qualified donor, we request:
1. You are able to make regular visits to our laboratory in Central area.
2. You are exceptionally healthy
3. You successfully pass two layers of diagnostic health screening conducted by the Asia Microbiota Bank
