If you had a recent breast cancer diagnosis and viewed the decision aid, you are invited to participate in a research project “Field Testing of the Begin Exploring Fertility Options, Risks and Expectations (BEFORE) Decision Aid” to evaluate the decision aid by completing an online survey.
The research project is led by Dr. Nancy Baxter from St. Michael’s Hospital in Toronto, Ontario. This survey is part of a larger study that is evaluating the use and delivery of the decision aid across multiple clinical sites in Canada. The survey will take about 2 minutes and can be completed at any time that is convenient for you. You have the right to exit the survey at any time for any reason and can skip questions you do not wish to answer. Your responses will only be saved once you submit them.

Note: No personal information will be collected if you choose to complete this survey. Your responses are completely anonymous and will not impact your care or employment. Do not identify individuals, or places, or use phrases that could identify you. You will not be compensated for your participation in this study.
By completing this survey you are agreeing to the following: As Survey Monkey’s servers are located in the United States, they are subject to the conditions of the former Patriot Act (now the USA Freedom Act). As such, we cannot guarantee that these files will not be accessed by others. The survey data will be saved for ten years in line with St. Michael’s Hospital policies. The data will be deleted from Survey Monkey and St. Michael’s Hospital secure internal computer server by a member of the research team after the study period.
