Sources of Exercise Self-Efficacy in Blood Clot Survivors

Hello:

My name is Julie Partridge, and I am a Professor in the School of Human Sciences at Southern Illinois University - Carbondale and I am also a PE survivor and a member of the National Blood Clot Alliance Sports & Wellness Institute. I am interested in learning more about blood clot survivors and exercise. We know quite a bit about the physical experience of blood clots and that exercise is often recommended for survivors, but we don't know much about the experience after a clotting event.

To learn more about this, we will be collecting information with an online survey with blood clot survivors (no matter how long it has been since you have experienced a clot). You must be at least 19 years old to participate. Your participation is voluntary; you do not have to participate if you do not want to and you may withdraw at any time, without penalty. In order to withdraw from the study, you may exit the browser and your existing answers will be deleted. I am the only person that would have access to the data and I will take all reasonable steps to protect your identity. The survey will take approximately 15-20 minutes to complete. A researcher will ask questions about your experiences with exercise as a blood clot survivor. You may choose to answer all, some, or none of the questions asked in the survey.

All reports based on this research and written by the researcher will maintain the confidentiality of all participants. No names will be used. There is very little risk of harm to you if you take part in this study, however, we hope that completing the study will allow us to learn more about the exercise experiences of blood clot survivors. Even after permission has been given, you can stop participating at any time and there is no penalty for stopping participation.
