Valley Medical Center Community Member Survey

Valley Medical Center is conducting a Community Health Needs Assessment (CHNA). We are asking for your input. This survey will help us better understand the health and social challenges you and your family face. Your responses, along with those of other survey takers, will remain anonymous.
