We hope you had both a professionally and culturally enriching experience in the United States. To comply with J-1 visa regulations, you and your supervisor are REQUIRED to complete a Final Evaluation. The survey should be completed prior to the end date of your J-1 training or internship program. Please make sure to answer the questions completely.
