The Coordinating Center (www.coordinatingcenter.org), a statewide, nonprofit care coordination organization with headquarters in Millersville, Maryland has received seed money from the Maryland Department of Health's Office for Genetics and People with Special Health Care Needs, to pilot VIPhysicians & Kids, a medical home program for pediatric practices for Children and Youth with Special Health Care Needs (CYSHCN).

The Maryland Department of Health's Office for Genetics and People with Special Health Care Needs defines medical home as an approach to providing comprehensive primary care that facilitates partnerships between patients, families, pediatric primary care and specialty care providers, social workers, educators and other providers.

In this partnership, families and providers work together to identify and access all of the medical and non-medical services needed to help children and their families reach their greatest potential.

The Maternal and Child Health Bureau defines CYSHCN as children who "have or are at increased risk for a chronic physical, developmental, behavioral or emotional condition and who also require health and related services of a type or amount beyond that required by children generally."

The Coordinating Center is looking for feedback from pediatric offices to inform the development of a pilot program, VIPhysicians & Kids. There is no cost to participate in the pilot program. We appreciate your participation in this survey.
