Healthy Alaskans 2030 Community Input Survey

Alaska’s State Health Improvement Plan, Healthy Alaskans, is updated every ten years. It is a process to make the health of Alaskans better overall by focusing on a set of common health goals. Healthy Alaskans 2020 identified 25 leading health goals that have been used by people working in the health field in Alaska over the last decade. Your input is needed to prepare for the next version of the State Health Improvement Plan, Healthy Alaskans 2030, with the vision of "Healthy Alaskans in Healthy Communities."

 ABOUT THE SURVEY

This survey is to find out what Alaskans think are the most important health issues in their communities and in Alaska. In this survey, you are asked to rate a set of health topics on their importance to you and your community. This input will help guide the selection of health focus areas for Healthy Alaskans 2030.

The survey will take about 10 minutes of your time. Your name will not be connected to your responses.

Thank you for your time and effort!
