Many people have already been helped by Cornwall's Workwell service - so why don't you request our help by completing this online form.

If you are off work or thinking about taking sick leave due to your health, we can arrange for our friendly advisor to get in touch and discuss how to make your work life easier or help you return to your job.

The support they can offer includes, building your confidence at work, support with understanding and managing your health condition and advice on how to access other services for support and training.

If you fill out the form below someone will contact you to discuss what support you need. Our advisors will never contact your employer unless you ask them to.
