CNF Roadmap To Resources Book- Nonprofit Partners

Child Neurology Foundation is creating our "Roadmap To Resources" resource book for our community of clinicians, families, and patient advocacy groups. This book is to share more about your organization or disease state, research happening in your area, or educational materials that your organization provides. There will be both a hard copy and a digital copy of the book. Please fill out the following questions if you would like to contribute to this content. Please note that ALL entries must be submitted to noleka@childneurologyfoundation.org by July 31, 2026 at the latest. Directions for entry submission are below. Thank you!

Graphic Requirements
Please submit one graphic that promotes the resource in its entirety.
Preferred file format: Print-ready PDF
Accepted alternate format: PNG at 300 dpi at final size

Your graphic should include:

Organization name and/or logo; Resource Name
Example: Child Neurology Foundation, Connected Care Network
Website / URL
Example: https://www.childneurologyfoundation.org
Description (1-2 sentences, approx. 15-40 words)
Provide a brief description of what the resource offers to families, caregivers, clinicians, or others.
Who is this resource for?
Example: Families, Clinicians, Caregivers, All
Notes / Additional Info
Include any helpful added context such as eligibility requirements, contact information, or a QR code.

File Naming Format
Please save all files using the following format:
OrganizationName.png or OrganizationName.pdf

Submission Notes*
Make sure all information is accurate and up to date
QR codes, contact information, and website links should be clearly visible and functional
Content should be clear, concise, and designed for easy reading in a printed format

*If created in Canva, you may share an editable link if CNF may make minor layout adjustments as needed.
