Study Title: Children and Young Persons Substance Use Needs Assessment
We are seeking your consent to send a survey to a young person (13-15) in your care.

Before you make a decision, we want to provide you with clear information about why we are doing this survey and what it will involve for your child or young person. Please read all of the information below. If you have any questions, feel free to email us at:
📧 community.research@essexrecoveryfoundation.org

If you are happy for us to send the survey to your child or young person, please complete the consent form at the bottom of this sheet.

What is the Purpose of the Survey?
The purpose of this survey is to better understand the experiences of children and young people (aged 13-24) in relation to substances such as tobacco, vapes, alcohol and drugs. The findings will help identify the types of support young people in Essex may need – whether or not they are currently using substances. This survey is part of a broader project that also includes interviews and data from individuals in treatment. Our aim is to gain a deeper understanding of how young people think about and experience substance use, the challenges they face and the ways they cope or seek support.
