Welcome healthcare professionals!

We’d like to create a free personalized learning module for you. This module will give you an approach to an older adult who says they’d like to die, so we want to include aspects of late-life depression, suicide and Medical Assistance in Dying (MAID). Realizing that everyone and every organization has different learning needs, we are hoping to build cases around your specific needs as part of this "bespoke", tailored education. That way each healthcare professional will obtain a specific customized education module to suit their own learning needs, which is ultimately the hallmark of bespoke education. 

To that end, we are sending you this survey to ask questions that will help us design your customized online module. Like other Geriatric Mental Health (GeMH) courses that follow a blended learning approach of online learning modules with live webinars, this online module will also be followed with a free webinar to consolidate learning.

In this survey we will collect some demographic data, but mostly want to know your specific learning needs.
 
The online module will be available from mid-October with the live webinar in November, 2017 (final date TBD). 

By continuing with this survey you are giving us consent and permission to collect and include your data about this "bespoke" education process. All information obtained in this survey will be held in strict confidence in a protected institutionalized server and only accessed by the project investigators and assistant. Furthermore, all names or identifying information will be de-identified for confidentiality.

There are also some pre-knowledge/attitude questions in this survey.  After completing the online module and participating in the live webinar you will receive another survey with post-knowledge/attitude questions. Approximately 6 weeks after the webinar we will send you another short survey asking about impact on practice.
