Registration Form for Vacation Bible School at CHSOS

Vacation Bible School is for children ages 6- 12. Children will be involved in singing, story time, Bible lesson time, craft time, snack (provided by CHSOS), outdoor fun/game time, and memory verse time. Each session will run from 9:00 am to 10:50 pickup time. At the beginning of each session, we need the adult responsible for the child to sign child in and write down who will be coming to pick them up that day with contact information. Adults are welcome to stay and wait for their child if they would like. Your children may participate in some or all of the sessions being offered. Please fill out the information below by no later than July 1st. Spots are limited so participation will be on a first come, first serve basis.
