Personal information collected on this form is collected by the Municipality of Temagami under the authority of the Municipal Act, 2001 and in accordance with the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).

The information is being collected for the purpose of assessing the extent of damage resulting from the June 30, 2026 significant wind event, determining whether to request that the Province of Ontario conduct an assessment for possible activation of the Disaster Recovery Assistance for Ontarians (DRAO) program, and administering any related municipal response. Information may be shared with the Province of Ontario where necessary for these purposes.
