This survey isn’t about what you know.
It’s about who you are,
what you think,
what you feel,
and what you believe,
 

It’s about your school,
your church,
your family,
and yourself.

 
Its about what is important to you in life,
what you do,
what you worry about,
what you enjoy.

Be honest in giving your answers. Your name will not be attached to the survey, so no one will know what you say. Your answers will be summarized together with the answers of other youth at church. All of these answers together will provide important clues to the interests, beliefs, problems and hopes that all of our students are currently experiencing. It’s important for your leaders to have these clues so they can do a better job of organizing our youth ministry program for you and your friends.

            For each question, decide which answer fits you best. Do not spend a lot of time trying to decide between two answers; it’s usually best to mark your first impression and move on to the next question. This is NOT a test! There is not right or wrong to any question. Just answer each question honestly.
