Overview

Thank you for your interest in participating in a key informant interview with the American Academy of Pediatrics Early Hearing Detection and Intervention (EHDI) Provider Education Center (PEC) program.

Please answer this short questionnaire to let us know information about your role in the EHDI system and the patients you serve. This information will be used to identify key informant interview participants. Note that due to project capacity and scope, not all interested applicants may be selected to participate in an interview.

While this survey confidential, it does contain optional questions about demographic and geographical information. We ask these questions as part of our commitment to culturally responsive and equity-driven practices that enable us to bring visibility to needs and strengths of people who continue to be minoritized and underrepresented.

Data will be stored on password protected computers and responses will be combined with other participants and reported in aggregate form. Any identifying information will not be disclosed through reports, publications or presentations related to the AAP EHDI PEC program. This program has been reviewed and approved by the AAP Institutional Review Board (IRB).

Completing this interest form will put you at minimal risk. This means that the risks are not expected to be greater than the risks a person may find in their daily life. You do not have to answer any question that you do not wish to answer, and you may stop completing the interest form at any time.
