Community Action Partnership of the Greater Dayton Area (CAP) is completing a Community Assessment, which involves asking questions about the needs of people in our service area. (Darke, Greene, Montgomery and Preble counties)

As a Community Action Agency, every 3 years we are asked to complete a survey of our community partners. The information we collect will help CAP plan and provide services to meet the needs of those in our service area and we want to know if CAP is currently doing a good job. The survey is anonymous—we do not collect names of those that complete our survey.

Please feel free to share this survey link with other service providers in your area.  https://www.surveymonkey.com/r/V9J7BQX

Respectfully,

John T. Donnellan
