Implementation of the Critical Care Clinical Network's Inotrope & Vasopressor Guidelines

This survey has been designed as an evaluation tool for the state wide release of the Critical Care Clinical Network's (CCCN) Inotrope & Vasopressor Guidelines. It lets CCCN know how many health services are using the guidelines, where they're being used and the safety of guideline implementation.

The survey should be completed at the following project stages:

- Beginning: the planning phase prior to guideline implementation

- Mid way : 1 month post guideline implementation

- End: 3 months post implementation

The survey will take less than 5 minutes to complete.

The provided information will only be used by CCCN.
