Good Shepherd Rehabilitation Family & Caregiver Survey

Good Shepherd Rehabilitation is exploring the needs of families caring for children with complex medical needs. This brief survey will help us better understand current needs and the potential value of medical daycare services in the Lehigh Valley and surrounding communities. Your responses will be combined with feedback from family listening sessions to help inform Good Shepherd's planning.
