1. Introduction
IPOPI, as part of the 3D-GATA2 research project, is carrying out this survey to learn more about the experiences of people living with GATA2 deficiency and their families.

If you are a person with GATA2 deficiency, a caregiver of someone with GATA2 deficiency, or a family member of a deceased person with GATA2 deficiency, we kindly invite you to take part. The questionnaire takes a maximum of 20 minutes to complete. Your answers will help make sure that the patient and family perspective is included in current and future research on this condition.

Nota:
• If there are different patients in the same family, please consider completing a questionnaire per person.
• Please note that if you are both a patient and a caregiver, consider completing the questionnaire as the patient.

Thank you very much for taking the time to complete this survey.
