Introduction
The community multi-disciplinary team (MDT) diabetes service comprises of diabetes specialist nurses, dieticians, podiatrists, support workers and wellbeing workers with diabetologist leadership.
The team work in community centres and GP practices across Northamptonshire and provide education, support and care to patients within the community who otherwise would have been referred for specialist advice and care in hospital.
NHS Nene CCG and NHS Corby CCG would like to hear about your understanding, awareness and views on the provision of its Community Diabetes Multidisciplinary Service.
