Planet Puberty Podcast: Feedback

FPNSW are distributing surveys to gather feedback on the podcast ‘Planet Puberty’. The information you provide, together with the responses of other participants, will be organised into a report. The report will be distributed among Family Planning NSW staff, and will be used to evaluate the podcast and direct our future work in this area.

You are being asked to participate in an online survey that will involve answering questions about:

·         how useful and relevant you found the information in the podcast
·         what you liked and disliked about the podcast
·         your recommendations for future podcast topic areas

The online survey is expected to take approximately 10 minutes to complete.

Participation is entirely voluntary and you may withdraw your participation at any time throughout the survey. There will be no associated consequences for withdrawing your participation or not participating in the survey.

 Your responses will be kept confidential and no information will be reported in any way that will identify any individual.

By agreeing to complete this survey you acknowledge that:

my decision to complete this survey is voluntary.
I do not need to answer all questions if I do not wish to.
I do not need to complete the survey if I do not wish to.
if I choose not to participate in this survey, or if I do not complete it, it will not impact my relationship with Family Planning NSW.

all survey responses are anonymous and will be kept confidential.
