Greetings! The Maryland Health Care Commission (MHCC) has engaged the Institute for Patient- and Family-Centered Care (IPFCC) to create a comprehensive Patient Family Advisory Council (PFAC) Guidance document tailored for Maryland Ambulatory Practices.

A PFAC is a formal group that meets regularly for active collaboration among the practice's clinicians, staff, and patient and family advisors on policy and program decisions. PFACs are a mechanism to invite patients and families in the practice to help improve and transform its operations/services by sharing their ideas, input and perspectives. Your participation will help ensure this Guidance document will effectively provide information, strategies and practical tips to ambulatory practices that want to implement a PFAC.

In order to get a broad representation of practices across Maryland, we are asking interested practices to complete this 10-minute survey. Thank you so much in advance for your time and interest!
