Smoking Cessation Survey

Research has clearly shown that smoking is a key risk factor in the incidence of the leading chronic diseases including cancer, stroke, respiratory illness and heart disease. A recent study by the Institute for Clinical and Evaluative Studies (ICES) has shown that amongst the four risk factors of smoking, poor diet, unhealthy alcohol consumption and lack of exercise, smoking has the most impact on hospital use.

The following survey is designed to assess the current involvement of health professionals in the Erie St. Clair Local Health Integration Network (ESC LHIN) region in the identification of smoking and smoking cessation treatment/support for their patients/clients.  This information will assist in addressing the needs of health care professionals and those patients/clients who are interested in smoking cessation as part of the future state care path model development.

Any information provided in this survey will be kept confidential and may be used for research purposes and/or published in a document.   Information will be reported on an aggregate basis only and will NOT identify any individual respondent or organization.
