Thank you for you interest in Universal Nutrition. 

At Universal Nutrition, we constantly strive to improve our products and services based on feedback from our customers. We greatly appreciate the feedback you will provide us by completing this survey. 

We are currently looking for feedback from international customers (those outside of the United States). As such, only individuals who reside outside of the United States, and who are 18 years of age or older at the time of survey completion are eligible to win a prize.

To show our appreciation for your time, the first 25 registered participants will receive a free t-shirt. In addition, we will randomly select one (1) Grand Prize winner. The Grand Prize winner will receive a special Gym Bag filled with at least $150 USD (MSRP) worth of unique prizes. Only entries submitted before February 8th will be eligible for the prizes.
