Vaccine Survey

This survey is intended for UUP professionals and academics, part-time and full-time, contingent and non-contingent. It focuses on COVID-19 vaccination and will provide information UUP needs to prepare for possible discussions with SUNY and the state about employee vaccination policies.

The survey takes five minutes or less to complete, and is anonymous. Please note -- to ensure anonymity, there is no way to pause and later resume or retroactively edit your responses once you have closed the survey.
