PARTICIPANT INFORMATION SHEET

WHAT IS THIS PROJECT ABOUT?
You are invited to complete an online survey about driving retirement for people with dementia or memory loss. The online survey will ask questions about the content of a new booklet about driving retirement for individuals with dementia or memory loss.

The booklet was originally developed for consumers and carers living in Australia. Your responses will be used to make amendments to the booklet and a new version will be made available that is relevant to consumers and carers living in the UK. You have been invited because:

- you are living with memory loss or dementia
- are a family member or friend caring for someone with memory loss or dementia
- are a member of an organisation which provides services to individuals living with memory loss or dementia
 
This project is being conducted in partnership between the University of Wollongong, Australia and Pennine Care NHS Foundation Trust, Greater Manchester.

WHAT IS REQUIRED OF ME?
If you choose to complete the online survey, you will be asked to read the Dementia and Driving Decision Aid Booklet and complete questions about its content. Completing the online survey should take between 15-30 minutes.

The booklet can be downloaded onto your computer, printed as a paper copy or read online.

POTENTIAL RISKS
Your participation in this research is voluntary. All responses will be collected anonymously and treated confidentially. All responses and any contact details provided will be stored on password protected computers and destroyed at a later date. Your choice whether to participate or not, will not affect your relationship with the organisation who forwarded you the invitation to participate in this project. 
 
WHAT WILL HAPPEN TO MY ONLINE RESPONSES?
Your online responses will be stored in a file on a password protected computer folder accessible only by the research team in the NHS and the University of Wollongong in Australia.  Your responses will be merged with the other responses we receive.  The merged responses will be reviewed as a whole group and we will use the responses received to develop the final version of the new UK dementia and driving decision aid. 
 
No individuals will be identifiable and every participant will be given a numerical code to ensure anonymity and that no individual is recognisable in the data.  We will also use the online responses to write newsletter items, reports, publications and presentations to promote the findings from this study.  No individuals will be identifiable in these outputs. Information will be kept for 5 years in accordance with the UK and the Australian law. After 5 years your online responses will no longer be accessible by the research team.
