Project ACHIEVE is a five-year study funded by the Patient Centered Outcomes Research Institute (PCORI) that aims to rigorously evaluate transitional care strategies employed by hospitals nationwide. It is also gathering information on the facilitators and barriers that hospitals experience in implementing these strategies in order to inform future implementation efforts. The American Hospital Association (AHA) is a key stakeholder and partner in this national study. We are asking our networks to take 10 minutes to complete this survey regarding barriers you experience to specific transitional care strategies.  The survey is anonymous unless you choose to provide your contact information in order to complete an in-depth interview. Aggregated, de-identified survey results will be supplied back to the Project ACHIEVE research team to help inform future implementation efforts.
