Patient Experience Survey

Section 1: About You

We want to hear about your health care experiences at your doctor’s office and with Upper Grand Family Health Team. This survey is voluntary. It should only take you about 5-10 minutes to complete.

The survey has 3 sections:
  1. Asking about your doctor's office
  2. Asking about your experience with the family health team
  3. Asking about your overall experiences with both

Your answers are anonymous and will not impact the care you receive. Your healthcare provider will NOT see any information that could identify who you are.


We will use your feedback to help us identify things that are going well and things we can work on that could improve your experience and care. Thank you for your interest in improving health care in the Upper Grand Family Health Team.
