GENERAL CONSENT AND RELEASE

The University of Florida is requesting your permission to use the capstone project you developed during your Master’s degree program in our publicity materials related to the program. This will broaden the reach of your capstone work by making it more visible and accessible to other teachers, and will also show other prospective students what they can achieve in our graduate programs. As you heard during the creation of your capstone, these projects are intended to be a resource for the educational community, and we hope that this effort will make them more useful to a broad range of teachers. We may also ask you to record a short video explanation and/or testimonial related to your project, to add more context and more of your voice to the posts. We encourage you to share the posts with your project on your own social media accounts, and you will always be credited as the creator.

In order to use your project and/or videos in our posts, we ask that you agree to the following release:
I agree that the University of Florida Board of Trustees (the “University”) may record my participation, appearance, likeness, and voice on any digital, analogue, or other device or storage medium, including DVD, CD, video tape, audio tape, or photograph (the “Materials”). I also agree that the University may use written, artistic, recorded, and any other products produced by me during the course of my graduate study, specifically including all materials related to my capstone project (the “Works”). I hereby unconditionally and irrevocably consent to the University’s use of the Materials and/or the Works for any legal purpose directly related to the promotion of the self-funded graduate degree programs administered by the University of Florida College of the Arts. I waive any right to be paid for use of the Materials or the Works, or to object to the use of the Materials or Works for the purposes specified under this agreement, including, publishing, printing, displaying, exhibiting, distributing, or otherwise publicly using the Materials or Works for the purposes indicated above. The University may edit, crop, retouch, or otherwise alter the Materials or Works to reveal my name and identity in the Materials or Works or do so by descriptive text or commentary. All intellectual property rights that are associated with the Materials are the sole property of the University. Intellectual property rights associated with the Works, excluding the uses agreed to above, remain the property of the undersigned.
