UF Impact Leaders Awards - Intent to Complete Form Survey

Are you interested in receiving your UF Impact Leaders Award?

Are you ready to study impact, practice impact, create impact, and tell your impact story (as explained here: https://go.ufl.edu/impactleaders)?

Students who complete the requirements above can look forward to receiving a certificate for your achievement, honors cords for graduation, as well a digital badge (https://www.credly.com/org/university-of-florida/badge/uf-impact-leaders-award).

The first step towards becoming a UF Impact Leader is to compete this "intent to complete" form so we can help you stay in the loop with opportunities to get involved as well as reminders & updates to help you reach your goal of receiving your UF Impact Leaders Award.

Later, after you've completed the requirements, in order to be awarded as a UF Impact Leader, you'll need to submit your UF Impact Leader Report by the deadline below:

-November 15 (for students graduating in Fall)

-March 15 (for students graduating in Spring)

-July 1 (for students graduating in Summer)
