It is recommended that you make and post your video application first. Then, gather all of the information you will need to complete this application. You may want to write your answers in a word document first and then copy/paste your answers into the application. Some of the questions are sensitive in nature, but we deeply value your honesty and willingness to share. All responses will be kept confidential and only seen by UChapter staff.
Also, please remember, the online application can not be saved.
Disclaimer: Before you begin, know that starting and running a chapter is a large responsibility and commitment. The purpose of this application is to help us get to know you better as well ensure that you are in a healthy place to establish and lead a chapter. Most importantly, we care about you and your health. If you feel that this responsibility is too much, that is okay! There are plenty of other ways to get involved with TWLOHA.
