(Required.)
9.If you are bringing guests, please enter their names.
This is to help us track potential attendance in the event that your guests also complete this RSVP form.
10.Please let us know if you or your guests have any food allergies or dietary restrictions we should be aware of. Lunch will be provided.
11.Where are you (and your guests) traveling from?
(City, State and Country if outside of the United States)
Due to the ASCO meeting in town, please be aware that it may be difficult to find an open hotel room during this event. IGCS is unable to locate additional rooms in the city at this time.

If you are considering traveling from out of town (and you are not attending ASCO to qualify for their housing block), you will want to ensure accommodation before making travel plans. We understand these circumstances may prevent you from attending.

We strongly encourage advocates and allies in the Chicago area to attend!
