UBI Follow Up Survey for Providers

* 3. What is the identifier code of the client for whom you just completed the UBI interview?
Note: this should be on the printout of the UBI. If this is not available, the code is built in the following way:
1. Client's initials _ _ _ (if no middle use the number 0), then
2. The day of the month client was born _ _ (if it's a single digit, use 0 then the number), then
3. The first two letters of the client's elementary school (if youth) or high school (if adult) _ _,
4. The first two letters of client's zodiac sign _ _

For example, if the client's name is John (no middle initial) Smith, born on June 9th, and he attended McAteer Elementary School, and he is a Sagitarius, your code would be: J0S09MCSA
Note: The code should be a 10-character combination of numbers and letters. If it is not, it will not be recognized by the survey and you will not be able to proceed through the questionnaire.
