SECTION I: Application Procedure

Please complete Section II (Club Information) and Section III (Player Development History) and email any additional information to Melissa Biniewicz at by December 1, 2012.

Please review Section IV (Academy Requirements) of the application to ensure your club is able and willing to adhere to all of the requirements of participating in the Academy.

December 1, 2012 – Last Day to Submit Applications
March 1, 2013 – Final Day to Receive Notice on Application Decision
March 15, 2013 - Final Day to Submit Performance Bond and Membership Agreement
August 1, 2013 – Start of Academy Program

*Clubs may be notified of their application status prior to March 1, 2013.
U.S. Soccer Technical and Administrative staff will evaluate the applications using the following criteria:

• History of Elite Youth Player Development
• History of Success in Elite Competitions
• Club Philosophy
• Club Style of Play
• Quality of Coaching Staff
• Club Programs for Zone 1 (5-12 year olds) and early Zone 2 (13-14 year olds)
• Geography
• Fundraising Efforts and Player Fees
• Overall Club Infrastructure (Facilities, Leadership, Administration)
• Impact on Existing Academy Members
Upon acceptance as a U.S. Soccer Development Academy club, your club will receive an email with detailed Rules and Regulation document and a Membership Agreement Form.

The club must return the Membership Agreement Form and Performance Bond by the March 15, 2013 in order to participate in the 2013-14 Academy program.
U.S. Soccer will review membership applications from previous years.

If a club has previously applied, please provide an updated form along with any additional materials to best describe the current state of the club.
