WCC Transportation Survey - Human Service Agencies

The Working Communities Challenge Transportation Team has designed this survey to document the transportation needs of the residents of our region. The goal is to better understand awareness around existing resources and what options need to be adapted to serve as many individuals as possible.

Your input around the experiences of your clients will assist our team in prioritizing our efforts in the coming months.

If you have specific individual clients facing transportation barriers, the last question of this survey provides space for you to list those individuals' general location (no names or street numbers, just street and town). The Transportation Team will do our best to find a solution to meet their needs. For this reason, we ask that you provide a contact person in case we have questions.

The information provided will not be released to the general public and only for internal use.

This survey should take about 10 minutes to complete.   We would appreciate it if you could answer the survey before June 25th, 2021.
