PAC Solicitation Authorization Form

*The Federal Election Commission requires an authorized representative of an UMA- member company to
submit written approval in order for the executive and administrative personnel of that company to
receive certain information and be solicited by UMA PAC, the association’s political action
committee.


I understand that my corporation may not approve solicitations by another trade association for the
same calendar year. I acknowledge that my authorization does not obligate me or other company
personnel to contribute.


* The company listed below hereby authorizes UMA PAC, a political action committee affiliated with
the The United Motorcoach Association, to solicit the company’s executive and administrative
personnel for contributions in calendar years 2015-2020.
