The Effects of Age-Based Bias on Levels of Mental Illness Stigmatization among Individuals Raised in the Southern United States

Participant Consent Form

This is an online survey on the effects of age-based bias on levels of mental illness stigmatization among individuals raised in the southern United States. It is being conducted by Ava Barnett, who is a psychology student at Truett McConnell. It should only take participants about 10 minutes to complete.

Participation
Participating in this study is purely voluntary. At any point, participants may choose to exit the survey or leave it incomplete. There will be no penalty for refusing to answer any questions, terminating involvement, or not participating whatsoever.

Benefits
Participants of the study may directly benefit by coming to a better understanding of their internalized stigmas against mental illness diagnoses.

Risks
The potential risks of this project are psychological, because it could evoke negative emotions in individual participants. For example, if individuals have mental illness diagnoses or have a history of mental illness in their family, participating in this survey could put a spotlight on uncomfortable memories.

Confidentiality
All answers supplied by participants will be contained in a private, password protected account on the SurveyMonkey website. Although your name is required for signing off on this consent form, it will only be seen by the primary investigator (Ava Barnett) and her research advisor. In the analysis and presentation of data, all information will be entirely confidential. No one, outside of the research team, will be able to see if you did or did not participate in the study.
