* 4. Saturday night we will have a social reception in the private on-site bowing alley, Wisteria Lanes. You do not have to bowl if you don't want to! This will be a relaxed social setting with a casual food buffet and cash bar from 6:30-8:30pm with festivities carrying on until 10pm. We do need an accurate headcount to purchase the correct quantity of food. Do you plan to attend the Wisteria Lanes dinner social?

* 5. On Saturday everyone will attend two breakout sessions in the afternoon. Please rank the sessions in order of your preference where 1 is your top choice and 12 is your least favorite choice.

* 6. IF YOU ARE BRINGING A CAREGIVER, one of their breakout sessions will be "Cushing's Caregivers". Please have them review the options for their second session and rank them in order of preference, 1 being top option, 11 being least interesting option.

* 7. We have had numerous requests to record this conference for people who are unable to come. We recently decided to add this element to the conference, so we will be recording the main sessions (not breakouts). The recording will not be broadcast live - it will become an online educational resource approximately three weeks after the conference.

The camera will always be pointed at presenters, but we would like to have everyone sign photo and video permissions just in case. Will this be ok with you?
