Welcome to the INTEGRATE olfactory disorders survey. This survey contains a series of questions on your ENT departments practice in the investigations and management of olfactory disorders. This will help complement the work being undertaken in the national prospective olfactory disorders audit.

Please note that completion of this questionnaire is a mandatory component of the audit, and will grant collaborative authorship on all future relevant publications.

Please note we only require one response from each centre participating in the audit, and if you are a junior doctor completing this survey we strongly advise you discuss the questions with a supervising consultant, to ensure accuracy in reporting.
