The following survey is regarding incidents of sexual harassment, discrimination or abuse experienced *and reported* while working as, in or with a government entity or individual in the state of California. The intent of this survey is to better understand and identify benefits of and challenges to existing structures for reporting and resolving incidents or harassment, discrimination and abuse.

Responses to this survey are anonymous unless users elect to provide contact information in their response. However, even if contact information is provided, it will not be publicly disclosed and will be used for communication purposes with We Said Enough only.

Please feel free to use the open ended comment areas to share information we have not asked for, additional details that you'd like to share, or to note where you are referring to more than one incident.
