Annual Patient Survey - Closing date 31st December 2020

Every year we seek the opinions of our patients about the services that we offer at Warmdene Surgery. The past year has been especially challenging for patients and for us, so your feedback is vitally important to help us learn and build on the changes that we have had to make. Your responses will be reviewed by the practice management team in conjunction with the patient group. This anonymous survey should take no longer than 5 minutes to complete. Thank you for your support and assistance.

Although there was a period when we were offering only telephone appointments, we were one of the first surgeries to reintroduce face-to-face appointments where it was safe to do so.
