What are we doing?


This survey aims to identify the strengths and resource needs of your community. Your feedback is anonymous. The information you provide in this survey is very important. It will be used to develop the 2028/2029 Community Action Partnership service plan to respond to community needs. This community needs assessment survey is being led by the County of San Diego, Health and Human Services Agency (HHSA), Department of Strategy and Community Engagement (DSCE), and Community Action Partnership (CAP). This survey will take approximately 15-20 minutes to complete.

What is CAP?

The County of San Diego's Community Action Partnership (CAP) is the region's Community Action Agency, funded by the federal Community Services Block Grant (CSBG). CAP's mission is to empower individuals and households toward socioeconomic advancement and increased access to resources through strategic partnerships.

What are we trying to measure and what will be done with the survey information?

This survey is required by the funder and is focused on capturing data and community input that will inform CAP's goals, programs, and services provided to individuals, families, and communities.
  • To support the County's vision for a ''just, sustainable, and resilient future for all,'' the information collected in this survey centers around the regional vision of Live Well San Diego's top five areas of influence: health, knowledge, standard of living, community, and social.
  • Your responses will allow CAP to identify what current programs and services are successful and what areas need improvement, including barriers to services, gaps in services, and ideas for future programs and services.
  • The information collected by this survey will be used to help guide CAP programs and resources.
  • The summary of the anonymous results will also be made available to the public and other service providers, including, but not limited to, other County divisions and departments, community organizations, and groups such as resident leaders working in the community.
  • The results will help identify opportunities for collaboration to meet community needs.
