This survey asks questions about your experiences with accessing and affording prescription medications used as disease modifying treatments for multiple sclerosis. The results of this survey will help the Multiple Sclerosis Foundation to represent the needs of the MS community as we advocate for access to and affordability of these vital treatments. Participation in this survey is voluntary and you may stop taking the survey at any time.
This survey is only open to individuals with MS and their care partners who reside in the United States and its territories.
To express our appreciation for your time, at the end of this survey you will be provided with the option to enter an email address to participate in a prize drawing. Seven survey participants will be chosen at random to receive an Amazon gift card. (The drawing will include one $100, two $50, and four $25 gift cards.) To be eligible to receive this prize, we must be able to verify the recipient is an individual with MS or their care partner.
