Dear Town of Brookline Residents,
The Town of Brookline, Department of Public Health and Human Services is inviting you to complete this survey about human services. Human services are services provided to people to help them improve their lives and find self-sufficiency through the provision of basic needs.
This survey is part of a broader community assessment and gap analysis and is intended to help the Town of Brookline determine strategies for better aligning human services with the needs of the community.
Gaps are defined as services that are needed but either do not exist or are inadequate to meet the needs of the community. Barriers are issues that make it hard to access services.
This survey is confidential. We will use the information you provide to gather residents’ insights and experiences as it relates to accessing human services. This information will help us plan strategies for reducing or eliminating gaps and lessen barriers. This survey will take approximately 10 minutes to complete.
